Lossless manual failover after quorum restored not possible?

Our quorum was lost then brought back online (i think).  Why couldn't we perform a lossless manual failover? Our config has a primary and a synchronous replica and a 3rd fileshare witness.  We had an issue with the local windows firewalls turning on via a rogue group policy on our servers.  This caused the quorum to fail. We tried restarting the primary server and it then had a windows issue that prevented booting (nightmare).  The secondary's status was 'resolving'.  However, once we disabled the firewall on the secondary shouldn't we have been able to do a lossless manual failover to make it the primary? Shouldn't the quorum have been online again with the witness?  We don't have many failures so we aren't very familiar with all the HA issues that can happen.
